TEAM ENTRY FORMS

Team entry forms can be downloaded below. A separate form must be completed for each team. Clubs should ensure players are listed in alphabetical order and that all names are spelt correctly. Completed team entry forms, together with one Grade Contact sheet per Club, must be e-mailed to chairman@nsnta.org before the closing date.
